Throughout season 1 you constantly see the Phantom docking with the Ghost nose first, despite the Phantom's MAIN hatch being located at the rear of the ship, seemingly leaving no way for the crew to enter. In Season 2 Ep 4 however, it shows that access to the Phantom from the Ghost is via a centrally located hatch in the floor of the Phantom, meaning that hatch could be used whether the Phantom is docked nose OR tail first. (There's no access via the rear loading door while docked, and no access via the belly hatch while landed.)
On several occasions, people may end up wearing suits that are not sealed and expose them to the vacuum of space.
Whenever the view from a probe droid's lens is shown, the word 'Playback' is shown in the in-universe script. Since it is a probe droid, it isn't playing back any footage; it is recording it.
On several occasions, the Phantom docks nose-first with the Ghost, yet the view from the Phantom's windows is facing rearward.
Captain Syndulla wears a faceplate if she expects the possibility of being exposed to vacuum. Since this faceplate does not cover her entire head, it would provide scant protection against the 2.7 Kelvin of outer space.
