- A child on a bike is hit by a car, the car then backs over his head, crushing it completely. This scene is very intense and incredibly disturbing.
- Melvin falls out of a building and lands in a vat of toxic waste. He then proceeds to burn and become deformed, becoming the Toxic Avenger.
- Nipples's eyes are gouged out.
- Many people and one dog are shot with lots of blood.
Profanity

- There are a lot of slurs used by the villains.
- Several uses of "fuck", "shit", and other profanity.
- Frank makes fun of Sarah saying, "that bitch is blind!"

- The drivers of a chemical waste truck partake in comical amounts of cocaine.

Frightening & Intense Scenes

- Bozo and his gang of psychopaths, while comically over the top, are extremely depraved. They are evil sadists who find sexual gratification in acts of horrific violence. Intentionally disturbing.
- The sequence in which a young boy is hit, then run over, by a car is very disturbing and upsetting.
- The scene in which the seeing eye dog is shot could upset people.
- Though the violence and gore is very cheesy and unrealistic, it still may disturb some viewers.
- This is a Troma film, and Troma films are notorious for throwing in many sexual jokes, gore, blood, death, homophobic, transphobic, sexist, racist and bizarre content into their films just for shock value.

Violence & Gore
- Near to the end of the movie, the Toxic Avenger removes the mayor's entrails. Very graphic scene.
- Frank's arm is ripped off and is put in a pizza oven.
- Leroy has a milkshake blender stuck down his throat.
- Rico's hands are deep fried and we later see the results.
- The top of a drug dealer's head is crushed with a set of weights.
- Toxie rips out Mayor Belgoody's intestines. He then tries to put them back in but dies in the process.
Profanity
- Julie uses racial slurs, a couple of characters are heard using homophobic slurs
