When Chakotay is trying to revive Janeway after the shuttle crash, he performs CPR. However, his hand placement is over her abdomen instead of her chest where he should be performing compressions. This was most likely done, as a lot of shows/movies have done, to avoid placing his hands on or near a "sensitive" part of the female anatomy. However, it ruins the realism of the scene.

When admiral Edward Janeway appears he is wearing a Voyager insignia comm badge. Having died 15 years ago he should be wearing a comm badge that matches TNG era with a round oval insignia instead.
After the shuttle crash and Chakotay is picking up Captain Janeway, the area where he is picking her up is nowhere near her left arm. However briefly she can be seen with her left arm raised up as if she was going to put her hands around Chakotay for physical support.
After her second "death" in sickbay, Janeway was able to make Kes sense her presence through physical contact; however, when Tuvok and Kes attempt to locate Janeway telepathically, she makes no attempt to touch either of them.
