Sex & Nudity

  • Very clear views of completely nude women in photos/paintings during prison road.
  • Full-frontal male nudity (clearly visible for most of the scene) can be seen for around 30 seconds when Teddy is walking by the prisoners. (1:10:43)
  • A realistic painting of a nude woman is seen.
  • A patient makes very crass sexual comments about a woman whom he maimed, implying that she wanted to have sex with him and that she wanted to be naked.

Violence & Gore

  • A man stabs another with a needle full of sedative, this isn't bloody but can be quite shocking.
  • When Teddy has a flashback to when he was at Dachau, the American soldiers line up the Nazi guards and open fire on the group. Graphic blood sprays. The Nazis are shown being shot as they are riddled with bullets and an overhead shot shows the aftermath.
  • A woman is shown soaked in blood , with dead children at her feet, covered with blood.
  • The film opens with a man being audibly seasick.
  • The aftermaths of many violent acts are shown and they are quite gruesome.
  • A man shoots another man twice in the chest. Blood sprays.
  • A man looks at paintings of people and various torture devices.
  • In one scene, a man is walking down a Nazi camp as he sees woman and children on the ground covered in blood.
  • A man face is very bruised and cut up from an offscreen beating.
  • A man jumps out at a person and strangles him, but he grabs him and punches him in the face, until somebody stops him.

Profanity

  • One use of "nigger."
  • 18 uses of the F word, 12 uses of "Goddamn", 10 uses of "Shit" (8 used as "bullshit"), 10 uses of "Hell", 6 uses of "Damn", 1 use of "bitch", 1 use of "Son of a bitch", 1 use of "Dick", 1 use of "Ass" 1 use of "cock", 6 uses of "Jesus", 4 uses of "Jesus Christ" 2 uses of "For Christ's sake" and 2 uses of "Christ".
  • Profanity is pretty infrequent, especially when compared to other Scorsese films like The Wolf of Wall Street, Casino, Goodfellas, The Irishman or The Departed.

Alcohol, Drugs & Smoking

  • A man is a recovering alcoholic
  • A man is seen taking pills with a shot of liquor to cure a migraine.
  • People are seen being offered and are seen drinking alcoholic drinks.
  • Several men smoke cigarettes, cigars, and pipes in almost every scene.

Frightening & Intense Scenes

  • The entire film is filled with disturbing, unnerving and scary images. Corpses of holocaust victims are shown spilling out of railroad cars. Bloody, dead bodies of children are shown up close. The main character has visions and dreams that become nightmarish. There are several jump scares, some more effective than others.
  • Disturbing images throughout, but nothing too extreme

Violence & Gore

  • A man has stitched scar on his face (aka. the fictional Andrew Laeddis). His face can be quite disturbing.
  • The dead bodies of two children are visble in water. It is implied that the mother drowned them. Not graphic, but very upsetting.
  • A man commits suicide, his body is later shown with blood all around it and one cheek is mangled from the bullet entering his face. This is not as graphic as it sounds.
  • A man shoots his wife, blood is seen dripping from her clothes.

Frightening & Intense Scenes

  • The scene where the man shoots his wife is intense.
  • The ending is very depressing.
  • Heading towards the end, it's revealed that the protagonist is the missing killer Andrew Laeddis. The buildup is incredibly intense, and the reveal is very shocking.
  • In two scenes, where Dr. Cawley shows the picture of Andrew's children dead bodies and in the next scene where he hold the bodies and line them on the ground can be quite disturbing for both adult and young viewers.
  • The movie is quite dark and disturbing.
