The hospital due for demolition (called "Castle Falls" in the movie) was "played" by the by-then long-disused Carraway Methodist Medical Center building in Birmingham, Alabama. Carraway Hospital first opened in the early 1900s as a segregated facility; it was desegregated in the late 1960s. A large star on the roof, originally a Christmas decoration but eventually kept on the building year-round, became a Birmingham landmark; the star is still visible in some shots in this movie. By the early 2000s the hospital had fallen into financial hardship; it closed in 2008 and the building was still unused by the time of this 2020 and 2021 film shoot, though in late 2021 a realty corporation announced plans to renovate it into a mixed-use development called "The Star Uptown," referencing the building's most recognizable feature.
Had shot for half a day (March 18, 2020) when it was shut down by Birmingham, Alabama's public health department because of coronavirus-imposed shutdowns. Filming resumed on October 28, 2020 for 17 shooting days.
