- VERY VIOLENT. the plot is effectively , you go down to hell, mankind is dead, and blood is fuel. you kill everyone and they explode into blood which heals you, and one robots arm falls off when hes trying to escape you, after a bossfight. and after his arm falls off, you grab it and it gives you a boom weapon.
- When you will kill an enemy, it will splatter and explode in blood and internal organs, you can also destroy the rest of the body remaining after you killed him. And the whole point of the game is to kill all of the demons and their blood is a way to get back your health.
- The game's violence is drastically toned down by the 1990s graphics.
- Additionally, there are in-game settings that let you turn off the blood and gore.

- The arena of the boss Gabriel, the judge of hell, is very disturbing, as It is entirely made of bloody flesh, with one eye in the center looking at you and in the distance you can glimpse a giant heart beating, implied to belong to the former judge, king Minos.
- While only one level (0-S) is explicitly attempting to be scary, many other levels in the game contain very intense and/creepy atmospheres. Notably, the layer of Heresy (Layer 6) is incredibly dark and intense, and the Gluttony levels (Layer 3) contain intentionally unnerving designs and music. If one suffers from fear of the ocean or sea creatures, The Leviathan (5-4) will be frightening as well.
