A tired toe that's been done to death. After the accident, Tully come to terms with her blame in the event - fair enough. What is soooo boring in this type of story line is Kate's "I can never forgive you line". Please, it's acknowledged that Tully's main 'crime' is letting Kate's daughter go out while she was meant to be grounded. The subsequent chain of events make that misdemeanour seem more significant, but that is the crux of the issue. Unless Kate would have been ready to throw away their friendship even if nothing had gone wrong it's just cheap and lazy writing that's been done to death on so many shows.
