- A slave owner forces two young slave girls to strip naked, their breasts, genitals and buttocks are shown. He proceeds to finger their vaginas.
- A man is shown publicly raping a young slave girl before a crowd of spectators. The man thrusts deep into the girl from behind, causing her whole body and breasts to shake during the rough sex. She attempts to hold back tears as the man climaxes inside her while the crowd cheers.
- A young slave girl is shown lying on her back, fully nude, crying in pain as a man thrusts into her. She is ordered to get on top of the man and they switch positions, her genitals are briefly visible. She then rides the man, showing her bare breasts and pubic hair. - Since it is set in ancient Rome, following along the lives of Gladiators there is a high amount of brutality and blood loss. The blood and gore is usually CGI but it is also very realistic due to the gritty battles and ruthless killings.
- Many slaves are seen beaten and whipped, due to disrespect.
- Gladiators use many deadly weapons to brutally murder their foes.
- A gladiator is seen having his head cut in half by a sword and fall to the floor a few yards away. You can see much blood and the inside of his head where the strike occurred.
- A man is stabbed in the mouth with a spear and his jaw is hacked off. This is shown in closeup and in slow motion. Extremely gruesome and gory.
Profanity

- "Shit", "bitch", "ass", and "bastard" are sometimes said.

- One main character Batiatus is shown beaten brutally and being peed on afterward. He is shown having bruises and cuts the day after.
- One Gladiator is forced to have sex with a female slave in public, who is married to the Gladiator's friend. This scene is highly emotional and tense.

- A very gruesome eye injury is seen inflicted in combat, with a close-up on the result. The character's eye is later shown crudely stitched-up.

- A young virgin slave girl is forced into prostitution as a sex slave, forcefully deflowered and brutally raped multiple times, only later to be executed in the arena for attempting to escape from her masters by severing the spine with a gladius. Her master rejects the opportunity to pardon her, and her fellow slave best friend who pushed her to escape and provided the means to do so watches on in horror.
