Sex & Nudity

  • Full frontal nudity is shown, albeit not explicitly given the low lighting and lower quality of graphics. Instances of sexual harassment, assault, necrophilia, masturbation, and sexual talk.
  • All the nudity and sexual stuff is hard to spot out because most of the game is in a dark asylum where you can't see a lot.
  • Mods to get rid of sexual content.
  • All nudity is either on dead or heavily mutilated people.
  • Full frontal detailed nudity is shown.
  • During a mission you go to a bath area and you see a character cleaning a dead body's stomach. He says sexual things like, "There we go. We clean your belly, clean your arms. Every little crevice until we find that key. I know one of you babies has it. There we go, shhh, shhh."
  • Dr. Trager is shown only wearing an apron, so his buttocks are shown.
  • There are two cannibalistic twins that want to murder you and they are completely naked with buttocks shown.
  • In the prison, there is a character masturbating in front of a toilet. While his genitals are not shown, you can clearly tell he's masturbating by his hand gestures.
  • A man in a prison cell tells you, "Nurse! I'm going to need some help getting clean... Nurse..." while laughing mockingly.

Violence & Gore

  • Chris Walker is shown killing a security guard by ripping his head off with bare hands. The scene is quick, and the headless body can be examined closer later on in the game.
  • This game has lot of violence, and disturbing moments.
  • When you get attacked, you either get your face punched or get your head ripped off. Blood covers the screen and the screen gets blurred with blood.
  • After you escape from Trager, you have to hide under a bed and you see another patient who yells to get Trager's attention. The patient then gets stabbed through the chest and you can see blood. Trager then tears the victim's chest out.
  • A PMC is seen impaled on an iron pole in a library filled with corpses and severed heads. He tells you to escape while you still can before dying.
  • During the first ten minutes of the game, Chris Walker grabs you while you're going through a gap. He calls you a "little pig" and throws you off the second floor. No blood is shown, but this scene is intense and is meant to scare the players.
  • Father Martin grabs you and injects you with a sedative. As you pass out, he shows you a security footage of a group of Tactical Team soldiers getting thrown around and killed by the Walrider (not graphic at all).
  • There are literally dead bodies everywhere in this game, and some of there have their organs splattered out, some of them are just body parts, and most of them are just lying down the floor with a puddle of blood underneath them.

Profanity

  • Some strong sexual dialogue throughout, not very noticeable but it's quite explicit.
  • Inmates use swears, and notes have swears. But you can change the language in settings
  • A lot of the profanity is not very noticeable.
  • Most of the swears are not noticeable.
  • Many uses of "fuck," "shit," "bitch," "bastard," "asshole," "damn," "hell," "son of a bitch," "crap," "ass," "whore," "tits," "balls," "dick" and "douche bag."
  • Religious profanities/exclamations include: "Oh, God," "Goddamn," "Jesus Christ," "Oh, my God," "For Christ's sake," "For God's sake," "Christ," "Jesus" and "My God."
  • Although the main character Miles Upshur doesn't speak, he expresses his thoughts in notes, which often contain heavy swearing.

Alcohol, Drugs & Smoking

  • There are some drugs shown in the asylum.

Frightening & Intense Scenes

  • This is one of the scariest games ever made.
  • The game can be upsetting and disturbing to some players.
  • The gore and violence are increasingly insane and realistic.
  • They aren't fibbing when they say this is one of the scariest video games of our time. Everything one display will send a shiver down your spine. With a chilling story, incredible soundtrack, and some really creepy enemies that you run into, and you have a recepie for a chilling and nothing less that iconic horror game.
  • The first proper jumpscare in the game is enough to send you flying out of your seat.
  • This game is meant to be scary, and is probably one of the most intense horror games.
  • The inmates are extremely scary, and most of the cutscenes are intense. This game also has an unbelievably grim and frighteningly realistic atmosphere, which makes the experience even more terrifying.
  • The story starts getting more and more disturbing.
  • The ending to the game is extremely disturbing and shocking.

Violence & Gore

  • During Trager's part of the game Miles has his fingers cut off. This is graphic and prolonged and although you look while it's happening it's still intense and the stumps are visible throughout the entire game afterwards. This is fairly moderate violence.
  • If you bought the Whistleblower DLC, there is a sequence that shows your character peaking through a small hole, and you see someone stabbing a sword through a chained up man's genitalia. They scream out in agony and and it's very bloody, their head then gets sliced in half with a electric saw. We don't see anything nudity though do his leg being in the way, however after this sequence, we get the players point of view which contains frontal nudity. ( See Sex and Nudity )
  • Dr. Rick Trager ties you by your wrist in a wheelchair and brings you through several hallways. Along the way, you see many of his victims. One tries to scream for help and Trager tells him, "Ssh... You weren't putting that tongue to any use anyway. Truth be told, I was just tired of licking my own stamps." You then arrive in a grimy industrial bathroom filled with blood and cockroaches. After a brief conversation between you and Trager, he mutilates 2 of your fingers. The actual finger cutting is not seen, due to Miles looking away, however the stump is seen for the rest of the game.
  • Chris Walker dies near the end of the game by the Walrider. He screams and you can see blood coming out of his body. There're crunching sounds and he's choked while squished into a vent.
  • You die at the end of the game by a group of PMCs. Blood shoots out of your body, while you moan in pain.
  • Near the end of the game, Father Martin is tied to the cross and one inmate sets him on fire to Father's wish, who screams in agony. His body is shown badly burned if you zoom in with your camera. Someone might find this sequence hard to watch.
  • Trager dies after he cuts off your fingers (mentioned above). You escape into an elevator and go down. Trager opens the gate and attacks you. Miles (your character) fights back and pushes him out of the elevator. Trager tries to get back in, but is crushed between the elevator and the floor. No blood is shown, however the sound of his bones breaking is.

  • Father Martin injects a sedative into your body.
