Sex & Nudity

  • Statues of women without clothing are shown at Cainhurst Castle. No nipples or genitalia appear to be shown.
  • There are some monsters dressed up, but if you kill them and look down at them, you'll see their members.
  • Some monsters appear nude and their rears exposed.
  • There is a sturdy monster that doesn't wear clothes and its molars appear and also rolls a lot and its hind organs appear disgusting.

Violence & Gore

  • Much more violent and gruesome than the Dark Souls trilogy.
  • The player can use weapons such as guns, cannons, swords, hack saws and handheld pile drivers to kill zombies, humans and monsters. To unlock different parts of the city in order to search for the cure, the player has to complete quests and solve puzzles to progress.
  • The overall level of violence is VERY violent. Combat is frequent and intense, and there are realistic blood effects where extremely over-exaggerated blood splatter stains the environment and characters. Sometimes there are so much blood effects onscreen the game will lag.
  • Players can perform what is referred to as a "visceral attack" where they use guns or sneak attacks to put an enemy off balance and then violently shove a hand into the enemy's abdomen or back, then rip the hand out resulting in massive amounts of blood splatter.
  • Many enemies can perform a grab attack that can severely harm the player character and is usually accompanied by very violent animation.
  • The player accumulates blood and gore on their face and outfit as they kill more and more enemies. The same effect can be seen on some boss enemies.
  • The player can collect a variety of gruesome artifacts to be used in dungeons. These can be anything from small goblets of blood to preserved fetuses. These items are accompanied by a large image and a description of the respective item.
  • For the most part, the violence in the game is brutal and visceral but not too graphic, though that is more than compensated by the extreme amounts of blood and gore during combat. Even then though, the ridiculous over-the-top blood sprays are extremely unrealistic.

Profanity

  • A female character who is a prostitute refers to herself as a "whore"
  • Mild profanities here and there (hell, damn, etc.) as well as mild insults (foul beast, wretch, etc.), nothing too strong.

Alcohol, Drugs & Smoking

  • Player can use various potions but none seem to have any sort of narcotic or addictive effect.
  • The game lore implies that people became addicted to the "blood healing" in the game.

Frightening & Intense Scenes

  • This is absolutely not a game for children. Teens who play this game need parent supervision.
  • Most of the enemy design ranges from creepy and foreboding to extremely disturbing and rather disgusting.
  • By far the most terrifying souls game to ever be released.
  • Rated R for strong bloody violence throughout including disturbing images, and disturbing elements.
  • The horror theme in the game is significant, as the action frequently takes place in dark foreboding environments and grotesque creatures are often encountered during game-play and in cutscenes.
  • The appearances of many of the monsters - and even the people - in the game are gruesome and frightening (A lot of the imagery was inspired by the works of H. P. Lovecraft).

Violence & Gore

  • One enemy rips out two of its own arms to use as weapons. The stubs will continuously spray blood.
  • One cutscene shows a character being beheaded. The scene is shown from a distance, with a large but quick blood spray and no sound effects.

Frightening & Intense Scenes

  • Amygdala's design might be extremely disturbing to most audiences (since they're all over the game but you can only watch them after killing a specific boss or having 40+ insight)
  • The horror in the game goes from almost a zombie like story to existential dread and cosmic horror which is very bleak and upsetting.
  • One character is referred to as a whore and has divine blood. A great one impregnates her and when her child is born it's a monstrosity. She's in despair, and in order for her quest to be completed the player character has to kill her and the child.
  • The Orphan of Kos is seen crawling out of it's dead mothers womb and starts crying. he also uses it's placenta as a flail. He is also considered the hardest boss in the game as it is rapid and unpredictable.
  • One of the bosses is a gigantic mass of body parts.
