Graham's mother is undermining his supposed diet by giving him treats and Phyllis is still unable to recognize son Gary, though he lives with her. Tony Beckton is still dominating agoraphobic Billy and young soldier Jack refuses to accept that he has post traumatic stress. The well-meaning Maurice is irritating Lorrie and Herbert's sister Amanda also thinks she knows best. At least the Cat lady April has allowed the council to clean her filthy home but in her private life Liz is still having problems as her estranged husband is not honouring access arrangements.

Community psychiatric nurse Liz does her rounds of patients, starting with elderly widow Lorrie, whose gentleman friend Maurice is maddeningly over-attentive. Obese Graham's condition is not helped by his mother's constant feeding him fatty foods whilst ex-convict Billy is reclusive and overly influenced by former cellmate Tony and erstwhile rock star Ray lives in the past - as does senile Phyllis. Last visit is to ageing thespian Herbert, who, without the voices in his head, lacks anybody to talk to. Liz is sympathetic to all her patients but she has problems of her own.
